Appeals Process

It is essential for ONE-T service providers to deliver services and make decisions fairly and accurately, according to the ONE-T plan provisions. As such, we maintain a clearly documented process for members to appeal decisions made by ONE-T service providers, including:

Health, dental and HCSA claims decisions by Canada Life;
Prior Authorization decisions for specialty drugs by Cubic Health; and,
Eligibility or billing decisions by Cowan, our third-party administrator.

This Appeals Process page explains the process and requirements for making your appeal to ONE-T, and this form is necessary for initiating the appeal.
