If your coverage through the ONE-T plan ends, you may be able to convert your life insurance into an individual policy with Canada Life, without having to provide proof of good health.

To qualify, you must:

• – Be younger than age 65, and
• – Apply within 31 days of your last day of coverage through the ONE-T plan.

If Canada Life approves your application, the maximum amount you can convert to an individual policy is $200,000 (including both your basic and optional life insurance).

1. 1. Obtain a quote from Canada Life; call 1.800.665.0551.
2. 2. If you decide to convert your insurance, contact Cowan at 1.888.330.4010 or 
3. one-t@cowangroup.ca.
   
4. 3. Cowan will fill out the “Group Life Conversion Privilege Notification” form and provide it to you.
5. 4. Send the form to Canada Life to finalize the conversion process.
